PRIVACY AND DATA COLLECTION POLICY
DATA COLLECTED AT TIME OF BOOKING
A certain amount of personal data is collected by at the time of booking. This is for the purposes of meeting Insurance Company criteria as well as for your health and safety at yoga. Health information is also used to tailor sessions to suit your individual needs.
You may update or correct your account information and email preferences at any time by logging in to your TeamUp account and changing your account and profile settings.
If you are not using the Online Booking system you will be required to a complete a health questionnaire (LINK TO HEALTH QUESTIONNAIRE TO BE PUT HERE) and this will then be stored securely for 7 years after your last class with me.
If you wish to alter any of your data, review how it is used or wish to request its return to you please contact Christine Yates on firstname.lastname@example.org or 07792676771.
If you do not wish to submit the information above then I may not be able to provide you with some or all of my services.
When booking either online, or on paper, you will be asked to sign a disclaimer. In doing so you are agreeing to your email and phone number being used by ChrisYoga to inform you of class changes, service updates directly relating to the service you have booked.
If you do not wish to submit your email, and therefore are unable to book online, you can book your class by contacting me on 07792676771.
CHRISYOGA MAILING LIST
When you purchase a service form me you will have the chance to “opt in” to receive updates on any new, future or alternative services that I offer. You can change this subscription preference at any time by emailing me at email@example.com. If you have an account with Teamup you can change your preferences at any time on your account settings.
I may, from time to time, use your phone number to contact you about urgent class changes or to inform you if a waiting list place has become available. If you do not wish me to contact you in this way please contact me at Chris@chrisyoga.co.uk
EMERGENCY CONTACT INFORMATION
Emergency Contact information is collected solely for use in an emergency, in order to contact your next of or give this information to the emergency services. This is the only information that would be shared with a third party, and only in the context of an emergency.
